| How do I request a certificate of insurance? |
|
All requests for certificates of insurance should be made in writing, and may be requested through our website or by faxing your written request to 952-831-0572. Your request should include 1) the name of the person or business requesting coverage, 2) the name of your company 3) the address of the requester 4) the telephone number 5) and the fax number or email address of the where we forward the certificate to the requester and send a copy to you.
